NEW EMPLOYEE EMAIL ACCOUNTS

To request email account, user ID and computer setup for a new employee, please complete the following 2 steps below:

  1. Open the "User ID Request Form" located at following link: userid_form_fillable.pdf. Fill in the required fields and print-out when complete.
    Please follow the recommended format for the User ID listed on the instruction page.
    a. This form must be signed by the Accountable
        Officer and by the User.
    b. Return the form to Campus Computing - A319.
    Once the user ID is approved, we will have the email account created.
    A sample user ID request form can be viewed by clicking on the adobe icon, the green form fields indicate what was completed online before printing it out to be signed.
     
  2. Enter a Help Desk request located at:
    https://www.neoinc.com/homepage/default.asp

    Please include all of the following information:
     
    Full Name of New Employee:
    Title:
    Department:
    Office Location:
    Mail Point:
    Phone:
    Supervisor:
    Starting Date:

    If the employee will be using a computer, please also include:
    a. where the computer is located
    b. the SAR# (or decal#).
     

Please remember that a User ID is required before our techs are able to set up a computer for your use.
