• Complete an application

• Bring copies of your disability documentation

• Bring the completed Documentation Coversheet

Pat will meet with you to review your documentation to determine eligibility.  Once approved, together you will develop a plan for academic accommodations.  A Memo of Accommodation will be sent to each instructor, outlining the specific accommodations you will receive for each class. Prior to the start of each semester it is your responsibility to contact the SDS office to arrange for accommodations.
