• Please complete only ONE survey this semester. By clicking on the appropriate buttons below, you may take the survey one of two ways:
  (1) on-line  OR
  (2) on paper by printing it out and dropping off your completed survey at the reception  desk in the Rotunda, in the evening drop box outside the Rotunda, or at the Cook Library circulation desk. You may also bring it or mail it to: Institutional Research, Mailstop C337, USF Sarasota-Manatee, 8350 N Tamiami Trail, Sarasota, FL 34243. Paper copies will also be available in the classrooms, at the Rotunda reception desk, and at the Cook Library circulation desk.
